How to create an anonymous Feedback board in Confluence?

Is there any possibility in confluence,

+ to create an anonymous feedback page?
+ to chat anonymously with a supervisor/leader?
+ to create surveys or polls and their reports for analysis?

would be great if confluence has solution for above-given feature.

Although Confluence doesn't have these as native features, you may find Apps from the Atlassian Marketplace that fit your requirements.

At least two of these requirements might be possible with ConfiForms.

I don't recall any native feature on Confluence that would allow quick creation of a feedback form.

We often refer to a feature in Jira that allows embedding forms on other systems, such as Confluence.

If you have Jira on your company you can take a look at the Issue Collector feature.
