Header doesn't increase in size / render correctly

I have a couple pages that I've tried adding headers to on Confluence 4.1.x to 5.1.3.

When I select the Header 1 from the menu bar (or use h1. ) my text is made bold but not actually converted into a header.

Those entries also don't show in a {table of contents} macro.

How can I correct it?

I periodically face the same problem. And there is no difference whether I change formatting of highlighted text or simply use 'h1. ...'.

Is there a way to fix this?

Does it makes any difference if you highlighted the text and choose from the drop down menu - heading 1?
