I want to create a form in confluence page, updated by users in confluence.
The data in the form will change as different people will add it at different levels , ideally it would be nice to show this in track changes mode if possible.
I think that the Confiforms add-on (https://marketplace.atlassian.com/plugins/com.vertuna.confluence.plugins.confiforms) would probably fit your needs.
Confiforms is based on macros and very hard to customize and have signatures.

I am not sure I understand this... What is very hard to customize in ConfiForms?
You can have own layouts for your forms and you can setup custom views over the stored data, there are many options here.
You can build views over the same data, but what looks very different: to show different sets of data and fields based on user type / security group (manager get's own view, while employee get's own)

No. The best way to implement forms in Confluence is via macros. It saves having to build an entire editor in a system that already has one.

Well, it depends on the license your Confluence is using - open source and community licensed Confluence installations could use marketplace plugins for free (when a vendor sells the plugin via the Marketplace)
