Button for creating subpages with a specific format

Hello

I would like to create for a collaboration with another internal team a confluence Page which startet a collaborated workstream.

a bit more concrete...

i would like to integrate a "Button" on the main Page which creates a subpage.
The subpage should have always the same format (i would like to have there some questions and checkboxes)...

The User which created the subpage and filled out the questions saves his input and in an ideal case i get directly informed that there is a new topic for me created...

is there any possibility to create such a page ?
I did not find a propper solution for it so far...
-the create by template Makro ist (at least as i tried it) not the right one...

What is it about the Create from template macro that doesn't meet your needs?

maybe you just need to create a template and use the Create button?
