Is it by seconds, minutes, hours or is it an absolute counter that only resets after you get the captcha?

You can set how many attempts are allowed before CAPTCHA show up. In order to configure that please check below documentation

So, the capcha only shows up if they are failed logins?

Yes, either done personally or by automatic services (if you login remotely via some tool/app/script).
