How to export a list of pages (and if possible their space), the date created and the author?

Question

I'm a pretty new user of Confluence, the company I work for is currently using Confluence 5.0.3.  I'm wanting to retrieve a list, (preferably in csv/xl) of the pages/blogs that exist (if possible their space), the date they were created and the creator.  If it is also possible to extract the last date they were updated and the name of the person who updated it within the same export, that would also be extremely useful.

Something like this (relative to confluence url) might be a good start:
