Can we perform Agile Maturity self-assessment survey in JIRA?

Question

We would like to perform Agile Maturity self-assessment survey for each team at regular intervals. Is there any way to do it within JIRA?

This option is available in Rally (CA Agile Central) with output as a spider diagram, anything similar JIRA may have?

Thanks!

Hey @Berry Kersten Did you find the file? I am also unable to access the file. We want to have similar assessment within Confluence.

@Shannon S Can you help with the file?
