Report on why issues failed

I would like to know if it is possible to preconfigure options to be provided once an issue has failed QA, e.g. Failed acceptance criteria, no description, etc. and then report on it.

How are you recording this in Jira?

Is there a custom field for it? A select list like "failed because" maybe?
