I got the same message.

I was trying to make a form out of a table in EXEL.

My workaround:  I saved the EXEL table into an earlier format.  That is, instead of FILE.xlsx (EXEL workbook), I saved it as FILE.xls (EXEL 97-2003 Workbook).

FInally got the program to read the file.
