If you're on Jira Cloud, the Backup manager can include attachments in the export:

1. Go to Settings > System.
2. In the IMPORT AND EXPORT section, select Backup manager.
3. Under Backup for cloud, select Create backup for cloud and tick the option to include attachments, avatars and logos.
4. When it's done, select Download backup file.

Keep in mind that backups with attachments need 48 hours between them, and Atlassian recommends staying within 25 GB of total data and 50–100 GB of attachments. Details: Export data from Jira Cloud

If you're on Jira Data Center, the XML backup only contains the database. A complete backup means backing up both the database and the Jira (shared) home directory, which is where the attachments are stored: Backing up data
