Create Issue popup doesn't close.

Question

In JIRA 5.1.5, I have one user using Firefox (Win) who sees the following behavior:

When using the New Issue Popup screen, he clicks "Create," and though the "Create Another" check box is not checked, the dialogue stays open; often causing him to create duplicate issues.

Anyone else seen this happen?  Suggestions?

any error messages on log file?

and also you can check error message by open firebug console on firefox
