Form input and implementation of forum

I am currently developing a one-stop webpage and need an input form to collect information from users. Most of the responses will be in text format, and I may also need checkboxes for some fields. Is there a way to achieve this without using the Confiforms plugin, as it is not available in my organization's internal Confluence? Additionally, is it possible to implement a forum? What is the usual approach to achieve this?

Are you looking to create a webpage in Confluence? I mean, Confluence is a knowledge-based tool rather than a replacement webpage (although it can be used to display customer-facing documentation).
If you could describe your use case a bit more maybe we could find some decent solution for this.

If you would use Jira Service Management, you could embed request forms into a Confluence page to get something like this (which can, essentially be a public form):

Related to the forum part, I believe you'll have to use some Marketplace solution, as, again, Confluence is more a wiki-style product than what you're looking for.
