The Premium and Standard subscriptions are attached to a Workspace, not the accounts themselves. If you're looking at an account with no boards on a Premium Workspace, it might be the case you created a new account.
With that in mind, a Premium workspace will be billable by the number of Workspace members and multi-board guests.
