how do I create a new issue form using scriptrunner

Using scriptrunner how do I create a new "Create Issue" form when user chooses a specific Issue Type -

Ya Screen can be different based on the issuetype selected.

@Florian Bonniec issue is .. a  customized create new issue form should be displayed to the user ONLY if the user has selected a specific issue type ( example - IT Ops) .. for all other issue types the standard create issue form should be displayed ... hope this clarifies my need.

I can create a new issues automatically BUT - here the form needs to be interactive .. that is the user can input values OR choose the default values for the fields.
