Unable to create issues of Request Type

We are unable to create "Request" type issues. The issue is specifically with the Request type "Feedback" . When we fill all the details selecting request type as "Feedback" and click on the "Create", Nothing happens.

As there is no error on UI when issue creation fails, you could capture the HAR file to check if you notice any error. https://confluence.atlassian.com/kb/generating-har-files-and-analyzing-web-requests-720420612.html
