Incorrect satisfaction - it appears automatically

I have a problem, that the satisfaction appearing automatically.
I asked my client why he give us 1 star 5 times in a row.
And he said to me that he didn`t choose anything by email.
So I don't understand how I need to control this. And why it is chosen automatically.
Please help me
