Major Incident' field should be available as Checkbox and once the checkbox is checked, 'Major Incident Number' Text field Should be asked as Mandatory field .

This implementation should be made for Type 'Bug

If you have ScriptRunner installed, you can implement this using Behaviors.

Another way to do it is to use third-party plugin called 'Dynamic Forms for Jira' from Deviniti
