Jira Workflow Button

I have a project with the Workflow button on Issues. Some of the statuses have (2) behind them. For example in the dropdown I have a status named "In QA (2)". I went to edit the Workflow, switched to "Text" and found the Step Name. It just reads though as "In QA". I went to edit on the Step and the Step Name and Linked Status are both "In QA".

Does anyone have any idea where the (2) is coming from? If it is a duplicate how can I identify where the duplicate is?

Kindly check in diagram mode, there might be two same transitions. Kindly remove "All" transition, keep other transition to remove (2)
