Confiform Action Buttons: Conditions Stopped Working

Hi Everyone,

We've been using our Confiform form now consistently for four months. Today we noticed a couple of things that have changed and am wondering if anyone knows why.

1. No changes have been made to the forms in months.
2. The Hide conditions no longer work - this controls when to display the action buttons
3. I have the TableView option to merge action buttons into one column. All of a sudden, the first action button listed shows with the field name as the column name whereas before, there was no column heading.

Can anyone point me in the right direction to start troubleshooting? I'd hate to re-do the forms that have been live since Q3'21. I know some updates to the add-on were available but in reading the release notes, it doesn't look like any of it would cause what we're experiencing.
