Create Issue with certain entries from confluence

I am looking for a way to create JIRA issues with 3 entries over a confluence form or page

We ask the user for component, summary, start date and end date. His entries are suppossed to be a jira issue in a defined jira project

That would work if there are not three more requirements.

A component needs to be selected and a start and end date.
and all three should be filled into the issue

You can also check the move section for jumping to Confluence to Jira and vice versa.
