Images that break without apparent cause

Hi, some of the users in our organisation have reported that images have disappeared several weeks after they were uploaded.

Is there an error or any 'quirks' that anyone is aware of that causes images to break and no longer display? 

Some of the documents are user guides that are needed for support so this is a big concern if the images will disappear without warning... 

By the way, the images do not disappear, they appear broken.

Can you confirm that the images are on the server?  I have seen it where images are uploaded and then security software on the server removes them and breaks the image.
