Where do I go to create a data entry form?

I am new. Have no how to access a forms   builder, so I can start building     a simple data entry form.

I would ne a text box where the person would select their name,.from a reportre drop down list.

Anther where the person would select a callers name from an employee dropdown list

Another drop down list where the reporter selects reason for the call.

 

The data should be added to an append table

Unless you really want to build a form in Jira... in this case you probably just need to create a project that has all the fields you want to collect and use it as a "form"
