In summary, here are the migration phases:

Assess: Understand where you are today and where you're headed
Plan: Determine the steps you'll need to take and develop your migration strategy
Prep: Get your people, data, and environments ready to migrate
Test: Ensure the migration will work and determine how long it will take
Migrate: Move your data and users to cloud and resolve any issues
Launch: Help users get set-up in cloud and retire server
