Request form with related/connected custom fields

Question

I`m looking for a feature/solution to create a intelligent and dynamic request form.

I`m working on a new request type, where customers need to fill in a lot of information (custom fields)

In order to make it more easier with focus on usability, I want to simplify the process, so customer only needs to fill in necessary information for his case.

I want custom fields to appear, when related data is filled in previous custom field.

Example: Request type "About you"

Custom field question: [Do you work?]
A) Yes
B)  No

Custom field question: [Do you exercise?]

A) Yes
B)  No

Then if "yes" is marked, another Custom field question should appear:

- Custom field question: [When do you exercise?]
   A) Before dinner.
   B)  After dinner.

In following picture I have illustrated how a specific value in the orange column, should bring up two additional fields in green.
