Adding feedback form to the screens/have a button enabled in tickets

Hi,

We are conducting training programs and tracking them using JIRA tickets. Would like to add a feedback form about the training in the ticket, so that trainee can submit it through the ticket. This should happen during a workflow transition 'Training Completed' and a screen opens up with Feedback form. How do we do this? 

Or It is also fine to have a button next to Workflow status in the ticket and make it available and mandatory when the status is set to Training Completed. 

Kindly help me on how to achieve this.

Ilakiya if you want you could add custom fields to the transition screen and collect feedback via the screen. This is covered in the Jira documentation here.

Without a custom built app, I don't know of a way to add a button right to the workflow status.

You might also like to consider the app ProForma (or ProForma Lite, a free version), which allows you to build forms that can be attached to the editor. You can use automation rules to automatically add the form to an issue when it transitions to a particular status.
