What kind of licences, is required for the approver role, e.g. the requestor's manager? Or can the business approval be performed in the customer role? Thank you!

They can just be Customers if you would like.

This functionality is NEW and hasn't been released in JIRA Server yet. It should be in the 3.2.0 release.
