Request not Working

Question

Dear members from Atlassian Community,

I'm finishing a Request form, but unfortunatly, this request is not beeing created, after I Click on "create", nothing is done.

Someone can help me?

I did a research and I couldn't find nothing about this.

I tried with other requests and they are working "OK",

But this Request is not.
