How perfroming jira transitions from confluence page on issue, {that transitions create subtasks}!?

I'm trying to implement a user view confluence page where I am suppose to show User details associated with JIRA-issue and Two transitions buttons. And when i select transitions that suppose to create subtask under the issue. How to achieve this!?
