We are using Jira and Confluence on the cloud. Is there a way to restrict or be able to setup approvals in Confluence without having to buy a tool?

Marc, if we are talking about approvals for page publishing, setting reviewers and notifying users about the current status of the page, the answer is no. Confluence does not implements this type of control natively.
We can achieve the desired results via third-party plugins, as you have pointed out. Here is a couple of them:
