Possible bug with pull requests and removed users

Trying to complete the pull request requires approval from a removed user.

It looks like you have a default reviewer for the repository in question. If you click on Repository Settings > Default Reviewers and from here you should be able to remove the deleted user as a reviewer.

Oh! My bad, I thought I had cleaned all those up.
