Is it possible to create an HTML form in Confluence that saves to the page?

Hello, 

 

I'm trying to create a Confluence page with a form on it that saves it's values in the Confluence page. I need things like check boxes and selects and they should remember their states and, ideally, also create page history, when being changed. 

Is this possible by using the HTML macro and aui forms? Or is there any way to achieve that?
