Regarding card cover images:

Please check the attached image. On a PC, the design created via a Power-Up is reflected on the Trello card. I am retrieving values from custom fields and reflecting them in the design.
* Note: All custom fields shown are set not to be displayed on the card itself, as they are rendered as part of the design programmatically.

However, on smartphones, the card cover does not show the full design; instead of looking like it does on the PC, only the card title is displayed. Is this the intended behavior?

Yes, this is expected behavior.

Trello’s card cover support on mobile is more limited than what you see on desktop, especially when the “design” is coming from a Power-Up and not just from a normal static image cover.
