How do I customize the "Track a Decision" window?

Hi all,

I'm trying to customize the Decisions functionality in Confluence Data Center to change the fields that users should be filling out.  The issue is that, no matter how I change the fields in the Decisions blueprint, the "Track a Decision" window that pops up still shows the original default fields and I can't find a way to edit the way any of this looks.

Is there any way to change the fields in the "Track a Decision" window as the space administrator?

That is a delivered functionality and you will not be able to change what is in the delivered product.  The track a decision screen that pops up can not be adjusted even if you update the template.
