Creating a dialog form that will create a page in confluence

Hi Atlassian Community,

How can I create a dialog box that contains form with fields like

title, status (dropdown single select), members (multi-select dropdown), and description

Once the form is saved using the dialog, it will create a page under a parent page.

A button in a parent page will display the dialog form when clicked.

Confluence is not intended to work that way, so you'll need add-ons.  Try "Forms for Confluence" and "Confiforms"
