I cannot replicate the problem since we are also getting successful submission and the information that we need from our customer. I even tried to submit on my end and it works well. So I'm not sure what's really going on if this is a Trello issue or no. For your assistance, thanks.

Klaviyo doesn't have an in-built Trello integration so this will surely be connected by something like Zapier or Make.
