How do I create custom fields for JIRA requests?

I am triyng to make a new request within Jira for our customers to fill out.

I need 9 custom fields, here are examples of what questions I need customers to fill out:

  • Primary Audience: What does your primary audience want? Why Will they care?
  • Goals: What are your primary and secondary goals? What are your S.M.A.R.T. goals?
  • Budget: What will your budget be?

You may have answered your own question here.  You need fields, so add custom fields to JIRA. 

If you are talking about "Service Desk Requests", you will then be able to add them to the request forms.

Yes, I am talking about Service Desk Requests.
