The most common solution for this problem assuming it is not affecting an entire project or instance is to run the Integrity Checker:

If you're having issues running the integrity checker through the UI (it may timeout due to proxy configuration) it's possible to validate against the database:

1. First check you have the Issue Transition Permission for the project where you are trying to create a issue.
2. Check if there is any condition on the transition which suppose to visible for you
3. Run the integrity checker for workflow
