How do I make headers for table of contents

I have created a table on my page with a header row but when I try to create a table of contents it doesn't list any of those headers, it just creates a blank TOC.  I can't seem to find what I'm doing wrong.

The table of contents macro will create a table of contents based on the h1, h2, h3, h4, h5, and h6 sections in the page. It doesn't do anything with table headers.
