Nope, not required to add workflow screen to any screen scheme. Check have you published the workflow after made the changes, also check to which issue type this workflow has been associated ...and attach the screenshot for the same ..if possible.

Ok, then please confirm, have you added the screen to that specific transition of the workflow. And also check the respective project that you have associated the workflow is correct because if you have been added the screen to particuler transition then it must pop up while transitioning the ticket.
