Checklist items have doubled when I have pressed delete option from the Card

Hi All,

I have once deleted the Checklist from the Card and when I created a new List and Card and press the Automation button to run the Checklist, then its number has doubled after the deletion. Suppose Checklist  has 6 items (tasks) and now it has populated 12 tasks (6 duplicate tasks).

Note: I have re-write the script/automation code for the Checklist in the Automation rules but the result has remain the same. Screenshot also attached for reference.
