"convert the item to a linked card" butler action creates random extra card

We've been using butler automations for the last few months, and it was working great until the last few days when one of the actions is creating random extra cards. The rule we have is:

"when an item starting with "[" is added to checklist "Tasks", convert the item to a linked card at the top of list "Tasks" copying labels and members, set due the next friday"

When you create an item with "[" at the front, it used to create a card that was directly linked in the item. However, it now creates two cards, one with the original title of the item, and the other with the url of the other card in it's name, both linked to the original card.

Not sure if others have experienced this, but looking to see if there is a work around as this rule was an essential part of our workflow.

Hey Frank,

I tried to recreate the issue on my own board, but I'm not having a second card created. Can you share an example of a card and checklist item that was showing this behavior please? That should help us pin down the issue.
