Create button on the board and associated screen

HI, Can someone please help? I am not able to get a particular screen open when clicking on the create button. The view screens area shows only a dot instead of the workflow name though I have properly setup the screen in the workflow transition. Please see attachment.

Figured it out for myself.  Transaction Screens are tied to Workflows where as Issue Screens are tied to Issues as Create, Edit and View Screens. This is initially very hard to understand. It would have been better if the software kept these two in a separate configuration screens.
