Can add a link to a Form in Jira Work management

I want to create a form to collect requests for designs, etc. 

What I am missing is the option to link a URL. How can I add that so it is recognized as a field and easily removed or edited.

Hello, Good day. URL custom field is supported in JWM form, you can create one URL field. https://support.atlassian.com/jira-work-management/docs/manage-your-form-fields/
