Having troubles with editing the system impact analysis

I am getting the following for some odd reason:

I have added "No CSI App ID" and yet it says that there are multiple entries.

Please assist

You have some app (or js based botch) that is providing some form of check on the data you are entering.  It could be a validator, a Behaviour (from Scriptrunner) or some other trick added by an app.

Whatever it is, It sounds like it is mis-configured, and you'll need to get your admministrators to fix it.
