Create a new Page (from a template) but site title has to be generated by input fields

We need this:
When a user creates a new page from a template by macro they should be get a text box for Name of the Customer, Street, City,... and after the user makes the input the new page should be generated with the site title from thte preasked values.

Any Ideas?

What you are describing is a Confluence Blueprint. Confluence includes a number of Blueprints, but if you wish to develop one for your own use, then it will require a bit of programming knowledge. Have a look at Atlassian Developer: Confluence Blueprints for information on how to do that.

As a workaround, if you aren't interested in developing a blueprint, you could create a page template. You won't be able to set the page title according to results that a user enters in a pop-up, but you can include a table in your page template and encourage the user to fill it out using instructional placeholder text.
