Confluence 'Questions' - Page Not Found

I recently installed Confluence 6.6.1, all working smoothly and the business are filling it with useful data.

We since purchased the questions plugin and simply cannot get it working, each time I install, I click 'Get Started' and receive the following screen, also note that the menu option at the top of the screen does not appear.

I have seen various articles stating issues with MySQL 5.7 and older versions of Confluence, advising to roll back but would have expected this to have been resolved throughout the newer versions of Confluence.

Any feedback/advice is greatly appreciated!!

Just to add to the above, I have attempted reinstall of the plugin, reinstall of a standalone instance of Confluence, clearing the plugins cache and still no change.
