I want to allow team members to flag problem Tasks and the person who unperformed on the Task. I do not what the under-performer to be able to remove record of the under-performance. Even if the under-performance is corrected, I'd like a record of the under-performance accusation and removal of the accusation to remain on record. Thank you in advance for helping.
Atlassian Team members are employees working across the company in a wide variety of roles.

This is not something that Trello can offer. We do have labels that can be used to flag certain cards, e.g. when a due date can not be met, however we do not offer any reporting as such in Trello natively.
