Images not defaulting as covers for cards

Good morning,

Whenever my team adds an image to a card, that image becomes it's cover. In the past, when they updated the image, they'd attach a new image and the cover would get updated automatically. Now, whenever they add an image to a card that already has a cover, the cover does not change. How can we make the card cover automatically change again to the most recently attached image?
