I'm wondering if anyone knows how I could send a questionnaire form to a customer so the answers they provide fill the appropriate fields on an existing card.
I can see how form data can create a card however I'm hoping to use a button in a card to send a form to request the data from the client and the returned data populates the card.
Any ideas would be awesome.

Welcome to the community. Cigdem from Hipporello here. Hipporello's Service Desk Power-Up offers many form templates and a really easy-to-use form builder. Once you build a form, you can choose to embed it to your website or publish it on a user portal provided by Hipporello.
When your client fills a form/questionnaire, the response automatically creates a card with all the form details. With some settings available, you can decide where you want the card to be created, with what card name and more.
It's also possible to make the necessary configurations if you want the form fields to update your custom fields.

This will work but not as pretty as you want. You have to create a web form with Contalist. Email the form with instructions to fill in the cardid in one of the form fields. When form it filled it can create a card and use Trello automation or pass the form data to an endpoint process it accordingly. Contalist webform has the ability to do that.
