Hello Djamel,

This is Jesse from the Statuspage support team. Welcome to the community. The incident creation section cannot be customized. It is set to provide incident statuses and component statuses and nothing more.

I would be curious to know what fields you would add if this were customizable. What would you do with a drop down list or a free text field?
